We are an out-of-network practice, not credentialed with insurance providers. Many insurance companies provide reimbursement for wellness services paid out-of-pocket. The following information regarding your session may be used to apply for reimbursement with your health insurance company. Please refer to this step-by-step guide to help you with this process.

We are not responsible for, nor can we guarantee the outcome of your reimbursement claim.


Step 1

ASK YOUR INSURANCE PROVIDER what information they need to process an OUT OF NETWORK CLAIM. If they need a DIAGNOSIS CODE, ask them for this code. We cannot provide that code, nor do we keep it on file. If your insurance provider requires the practitioner’s TAX ID NUMBER, please have them call us directly for that number. 206-902-7608


Step 2

SAVE YOUR FOLLOW-UP EMAIL It has all the information we can provide. Below is an example of the information provided in your follow-up email. It is sent the same day as your appointment. Search your email for the heading; Check In & Reimbursement Info from sender, Nurture Wellbeing. We do not take requests to replicate these at the end of the year.
